Girls who changed there name...

on the form you filled out for SS, did you include your parents SS#? Someone told me I had to and someone told me I didn't. The instructions say you don't have to have it, but I don't want to go down there and find out that I do need it.

I didn't have to fill in my parents social security numbers (I went to the one in West Babylon). GL!

you dont need the numbers

i think the form says something about needing them if you are under 12 so people get confused

you can google SS-5 and the form you need will come up!

Yeah my dad kinda freaked (understandably) when I asked him about it!
So I am going with the form filled out tomorrow at Noon and hopefully hit the DMV as well....keeping my fingers crossed I'll be done by 4!!!

I had to...I went to the Mineola office. I had to call my mom when I was there to get their ss#'s. Someone told me I didn't need it and sure enough..it was right there on the form...HTH

if you read the form, you only need your parent's ss#s in certain cases, the only one i can remember off hand is if you are under the age of majority. when i went to the mineola office, i didn't need my parents' ss#'s.
