Help with addressing invites (2 questions)

1. How should I address invites where the wife has kept her maiden name, and 3 of the children have the father's last name and the other child has the mother's last name? Would it be: Mr. John Doe, Mrs. Jane Smith and Family address 2. My FBIL and his fiance live together and he has a daugther from a previous marriage with his last name. How to address? Would it be: Mr. David Jones Ms. Jennifer Allen Miss. Cindy Jones (daugter of Mr. David Jones) address
