For Social Security card, you just bring your marriage license and another form of ID to the office, and you can do it right there. You'll get it back in 2-4 weeks. For DMV, I believe you have to do it in person at the office. I brought my new SS card, my marriage license, and your old license. If you have a NY license already, that's all you need. If you don't have an NY license, you need another form of ID like your passport.

Re: name change question?

I forgot to add that you should bring your old SS card to the SS office, too, just in case they ask for it. But they didn't take it when I was there - they just took a look at it. For the DMV, they took my old license.

Yes you will need your old SS, your mariage licence (certified copy), another form of ID (they may ask for 2), SS form.

For DMV, new SS, copy of mariage licence, old driver licence, name change form.

Also, if you own a car, there is another form for the title.

Re: name change question?

The only thing I needed at the SS office was my drivers license, marriage certificate, and the SS form. They never asked me for my old SS card or another form of ID. The SS form should tell you everything you need.

They asked me for both the old card and another form of ID at the office in Melville.

Check the SS website for their requirements.
