A Whole Bunch of Questions about Addressing Invites

First of all, my invites do not have inner envelopes, only outer.

1. Do I write, 'Mr. & Mrs. John Smith' or 'Mr. and Mrs. John Smith?

2. Do I write, 'Mr. Sam Johnson & Guest' or Mr. Sam Johnson and Guest'?

3. Do I write an address with an apartment number as:

123 Main Street, Apartment 2R
or
123 Main Street, Apt. 2R
or
123 Main Street, #2R?

BYW: I was formal on my outer envelope and informal (for family and friends) on my inner.

ex.
outer: Mr. and Mrs. John Smith
inner: Uncle John and Aunty Betty

Outer: Miss Susan Doe
Inner: Susan Doe or Susan
(if it was my parents friends I did Miss Susan Doe)
