I saw Dr DeMento in Garden City - don't know if he takes your plan but a couple of people recommended him to me.

Dr. (Jordan) Zuckerman

He's right across the street from Bay Terrace. He took my old insurance, my new insurance and DH's insurance so I'm pretty sure he's not too picky.
